Do not include illustrations -- nothing will mark you as an amateur faster. Print your stories out on a quality printer, and use regular typing paper. Any kind of special paper, such onion skin or stationery, will, again, only make you look like an amateur.

As you see above, put your name, address, email address, and phone number in the upper left margin and the word count in the upper right margin. Tab down a few lines and type the title, centered. Use 12-point font and double space.
